Dual Diagnosis Program

Case management services are provided to individuals with mental illness and developmental challenges who are ages 16 years or older.  Services are designed to promote recovery and independence while providing appropriate community based support.

While services are provided for as long as the person requires them, the goal of the Dual Diagnosis Program is to promote recovery and independence.

Please click the following link www.northwestaccesspoint.ca in order to complete an on-line referral form for case management services.  Clients may self-refer or may be referred by another person/agency (ie. health care provider, family members etc.).
